How can I apply to become a member?

To become a member of the M.F.V. Panacea, you need to follow these instructions:

  1. Fill out the required data in the registration form and pay the membership fee.

  2. Sign the SEPA-form and scan it.

  3. Wait for the confirmation of your registration as a member.

  4. Enroll for the Introduction!

1. Registration form and payment

Via the registration form, you give us the data we need for your registration. When you filled it out correctly, click 'send' and you will be directed to the webpage for payment. The costs for membership at the M.F.V. Panacea are €15 per year, and you pay for the duration of your study at once. This makes a total of €90,- (transaction costs excluded) for the entire membership during your studies. When you have payed the membership fee, we will receive your registration data.

2. SEPA-form

For some of the events organised by the M.F.V. Panacea, the costs will be withdrawn from your account after the event took place. This is mostly for events with a low entrance price, but sometimes also for events for which the price isn't fixed. To make sure we can withdraw this from your account, you will have to permit us. We therefore ask you to fill out a 'SEPA'-form for continuous payment authorization.

The SEPA-form will be send to your e-mail address after step 1. Print the form, fill it out, sign it for authorization and scan the form. Then send the form to inschrijving@panacea.nl. Sending the signed SEPA-form is a requirement to become a member!

3. Confirmation

After we received your registration, we have to check everything. Because of the number of new members, this could take some time. That is why it can take a couple of days before you receive a confirmation of the registration as a member of the M.F.V. Panacea. You will receive this confirmation via e-mail. 

4. Introduction and other activities

When you have received the conformation, you are a member of the M.F.V. Panacea, which means you can log in at the website and enroll for activities! You can also enroll for the introduction, don't miss it! You are now able to buy your study books and lab coat. Also, you can now view the photos of previous events and find other members.
